Who should use this form?

Anyone to whom the following has happened:

• Your name has changed

• Your address has changed and you want your new address to appear on your new driver licence
(if it is only your address that has changed and you do not want the new address to appear on your driver licence, please update your address online or call 0800 822 422. There is no fee involved)

• Your appearance has changed significantly from that shown in your current driver licence and/or driver identification card and a new photograph is required

• Your driver licence and/or driver identification card has been lost or stolen or destroyed

• Your driver licence and/or driver identification card has been defaced or mutilated or become illegible
